Our first equipment fitting day is this coming Saturday, July 16th from 10am to 4pm at the ASA Wrestling Building.  Please come through the front door.  Registration paperwork and payment will be checked and verified at the front desk.  Equipment will be issues to players that all paperwork and paymemt have been handed in.  There will be forms available to fill out if needed.  If paperwork and payment is not completed, the player can be fitted, but not given any equipment, we will log the sizes and hand it out when everything is handed in.

All Registration Paperwork and registration payment is due before equipment can be issued and your child be able to practice. If you have any paperwork left to turn in, please turn it in at the Football Equipment Day on Saturday, July 16th or Sunday, July 24th, whichever you come to. I would strongly suggest coming to the first fitting if possible. Coaches will not deal with paperwork on the first day of practice. This is to be taken care before your first practice. Here is the list of required documents:

• Pop Warner Participant Contract And Parental Consent Form - (2 pages) Signed by parent & player. *** This needs to be the orginal signed copy ***
• Pop Warner Physical Fitness & Medical History Form - (2 pages) *** This needs to be the orginal signed copy *** - No other physical form can be used in place of this form.
• Parent Athlete Questionnaire - Not needed if registered on-line.
• ASA Volunteer Agreement - This is an ASA form and can be the original or photo copy.
• ASA Code of Conduct - This is an ASA form and can be the original or photo copy.
• 2010-2011 Year End Report Card - must show all 4 marking periods. If your child has attended multple schools for the 2010-2011 school year, report cards from all schools are required.
• Copy of Birth Certificate - Not needed if played with ASA last year. This can be a photocopy, but has to have the raised seal visible on the copy. No mother's copy allowed. The full name that is on the Birth Certificate has to be the same name on all the paperwork.
• Registration Fee - must be paid before equipment can be issued to a player.
